To keep up with our continued growth, we are seeking a practical and adaptable Complex Care Coordinator to help support, facilitate and co-ordinate services for individuals with a severe mental health disorder in the Wide Bay area.

The role

  • Provide quality stepped care solutions to people who have been diagnosed with a severe and persisting mental health condition.
  • Provide key support to our clients by building on strengths, holding hope and enhancing resilience to promote recovery.
  • Deliver a range of services with a focus of improving social, occupational, personal and clinical outcomes through appropriate networking and integrated care solutions.
  • Liaise closely with family and carers as appropriate, regularly review the person's mental state and, where appropriately qualified (e.g. mental health nurses), administer prescribed medication, monitoring any potential side effects
  • Provide consultation and liaison with primary health care providers, acute health organisations, and rehabilitation and support services.
  • Improving pathways and links to other health and clinical service providers
  • Provide psychoeducation, and health information as required and where appropriate, assist in addressing the physical health inequities of people with mental illness.

What we’re looking for

  • Current Nursing and Midwifery Board of Australia Certificate of Registration, or;
  • Social Worker/Occupational Therapist with Mental Health accreditation (or working towards), or
  • AHPRA Registered Psychologist
  • Substantial experience and knowledge in client and risk management, working with vulnerable clientele from diverse backgrounds.
  • Demonstrated ability to practice within a recovery framework and early intervention framework (when working with young people) in accordance with the Standards of Practice For Mental Health Nursing.
